Sprain Grade System:


Grade I (Mild):

  • Marked loss of function and joint instability.

  • The tissue is completely torn, with the two ends. possibly separated.

  • May require surgical intervention.


Grade IV (Severe/Complete):

  • A more severe form of tissue tearing.

  • The tissue is completely torn with a greater degree of separation between the torn ends or tearing off the attachment site of the bone, or tearing a portion of the bone off with it.

  • Significant joint instability and loss of function.

  • May require surgical intervention.

What are common symptoms usually associated with a sprain or strain?

1. Pain

  • Usually immediate or develops shortly after the injury

  • Worsens with movement or weight-bearing

  • Localized to the injured area


2. Swelling

  • Caused by inflammation and fluid buildup

  • May appear within minutes to hours


3. Bruising / Discoloration

  • More common with moderate to severe sprains/strains

  • Indicates bleeding under the skin


4. Tenderness to Touch

  • Sensitive or painful when palpated

  • Helps identify the exact tissue involved


5. Limited Range of Motion

  • Difficulty bending, straightening, twisting, or bearing weight

  • Stiffness due to pain and swelling


6. Weakness

  • A strain (muscle/tendon) may cause noticeable weakness

  • Harder to perform usual movements (lifting, pushing, walking, etc.)


7. Instability (More common in sprains)

  • Affected joint may feel loose or unstable

  • Especially common with ankle and knee sprains


8. Muscle Spasms (More common in strains)

  • The injured muscle may tighten or “grab” involuntarily
